INSURANCE

We are participating providers for most of the insurance plans available in our area. Many plans will cover some of the cost of your treatment. The procedure and fee schedule varies significantly from plan to plan. Many plans require pre-authorization before your first visit. If possible, please be aware of the details of your particular plan. As a courtesy, we will contact your insurance company to obtain benefit information and pre-authorization but you can assist in the process by understanding what your benefits are.

Any copayment or coinsurance is due at the time of the visit.
